Job Summary
Coordinate high-risk patients using Epic population management to track attendance, identify barriers, and facilitate timely care initiation. Maintain a caseload by documenting all encounters, scheduling follow-ups, and ensuring complete, up-to-date care documentation accessible to the team. Collaborate with clinical staff, external partners, and community organizations to provide warm hand-offs, address care barriers, and participate in multidisciplinary case conferences. Deliver outreach and event support to promote patient engagement and health equity across DAP Health clinics in the Coachella Valley and San Diego County.
Required Qualifications
- Three years' experience in a clinical setting or community health setting
- Three years' experience in care coordination
- Knowledge and experience of health system navigation, connecting patients to social service support agencies and resources
- Knowledge of State programs
- Familiarity with EHR and patient tracking systems
- Current BLS certification obtained through the American Heart Association or American Red Cross
- Ability to lift up to 24 pounds
- Occasional travel between DAP Health locations

- Bilingual (English/Spanish) preferred with sensitivity to linguistic and cultural needs of patients
