Wellness Navigator

Remove personal and environmental obstacles to health care access by working closely with primary medical providers, clinical teams, and agency staff to proactively manage patient care. Provide case management, education, care coordination, systems navigation, advocacy, and support primarily in home and/or community settings. Engage, educate, and offer support to individuals, families, and caregivers to connect them to prevention, diagnosis, timely treatment, and follow-up. This role supports CBI's integrated behavioral health programs across Arizona, helping patients experience recovery while adhering to a "no wrong door" treatment approach.
