Job Summary
Conduct Medicare Annual Wellness Visits under physician supervision to identify personalized prevention plans with objectives and goals for beneficiaries. Develop written, age-appropriate individualized prevention plans addressing psychosocial, behavioral, and physiological risk factors alongside USPSTF recommendations and vaccination schedules. Educate patients and families on disease processes, treatments, and risk factor modification strategies while performing nursing functions such as providing immunizations and collecting laboratory specimens. Collaborate with physicians and team members to ensure referrals and follow-up care, maintain knowledge of quality measures, and ensure adequate documentation in the EHR. Promote wellness and illness prevention using clinical nursing knowledge of disease processes and risk factor identification.
Required Qualifications
- A minimum of three (3) years relevant clinical experience
- demonstrates leadership and autonomy in nursing practice
- Completion of an accredited LPN program
- Licensed in both New York and Pennsylvania
- current license as a Practical Nurse in their state of practice prior to position start date
- Must be able to obtain additional state licensure within 6 months of hire
- Must be able to travel
- Maintains relevant nursing knowledge of evidenced based strategies related to health promotion and illness prevention
- Maintains thorough knowledge of quality measures related to such evidenced based practices
Desired Qualifications
- preferably in an ambulatory healthcare setting
- Preferred experience with care management/utilization review
- payer knowledge
