Job Summary
Conduct comprehensive onsite intakes and screenings at hospitals, courts, and housing sites to identify prospective participants for Outreach programs. Provide educational workshops and assist clients in procuring necessary documentation while maintaining professional presence at assigned referral sources. Work with the Manager of Community Partnerships to expedite admissions and ensure warm transitions to the Outreach program. Maintain contact with referred participants until admission is complete. Achieve monthly and quarterly targets for the Wellness Liaison role. Full-time non-exempt position with a driver's license and bilingual English/Spanish skills preferred.
Required Qualifications
- High School Diploma/GED Equivalent
- 1 -2 years of work experience

- Knowledge and experience working with at risk populations
