Job Summary
Lead, manage, and ensure accountability for clinical resident care services by delivering a 1440 experience and optimizing performance. Own relationships with the Executive Director and Sales Team to market wellness, increase visibility, and build trust within the community. Perform admission and ongoing evaluations of residents every six months, initiate care conferences, and set measurable goals addressing physical, mental, and psychosocial needs. Manage revenue for Wellness, Evaluations, and Care Conferences while ensuring compliance with state regulations and auditing. Review incident reports to determine assessment needs and address personnel issues. Communicate emergency policies to staff and obtain written orders for care plan changes. Verify MARS/TARS and coordinate with pharmacy for medication accuracy. Work a flexible schedule including on-call, weekends, and holidays when necessary.
Required Qualifications
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) registered within the State
- CDP required prior to or upon hire within 90 days
- Available to work a flexible schedule including on call, weekends, and holidays when necessary
- Ability to occasionally lift and/or move up to 25 pounds
Desired Qualifications
- Registered within the State preferred
- Assisted Living/Memory Care experience preferred
- Leadership experience preferred
- Proven ability to manage revenue and financials successfully
