Wellness Director - LPN Minimum - Salaried Position - On Call Rotation

Responsible for medication supervision/administration and resident care oversight in a senior living setting, coordinating with the Memory Care Coordinator, pharmacy consultants, and external providers to ensure state regulations and policies are followed. Duties include supervising medication administration, monitoring responses to treatment plans, implementing and maintaining care plans and resident incident reports, conducting monthly self-medication reviews, and coordinating with home health, hospice, physicians, and pharmacies. Provides communication with residents’ families, administers training and coaching for team members, and participates in performance counseling with the Executive Director. This role requires being on-call 24 hours and rotating on-call responsibilities with the Executive Director, with a focus on ensuring safe, compliant, and compassionate care for residents across memory care and general resident services.
