Wellness Coordinator

Job Summary
Conduct intake interviews to determine individual needs and establish case management plans for connection to income, housing, and recovery coaching. Serve as a consumer advocate in outpatient and inpatient settings while coordinating transportation to treatment activities and monitoring progress toward established goals. Plan and implement sober activities, educational events, and awareness programs for the community, establishing relationships with partners and law enforcement to identify individuals with OUD. Maintain confidential files, track customer data, and report program outcomes to the director and funding sources. Occasional nights, weekends, or overnight travel may be required.
Required Qualifications
- High-school diploma or equivalency
- At least two years of experience in social services, counseling, or related field
- Demonstrated knowledge of community services programs and community resources
- Valid Driver's License and be insurable under agency policy
- Occasional nights, weekends or overnight travel out of county and in county travel may be required
- Extended hours may be required during critical workloads
- Requires standing, bending, reaching, sitting in working with office equipment such as computer, telephone, etc
- Occasional lifting up to 50 pounds
Desired Qualifications
- Undergraduate or associate degree or undergraduate work in human resources, social services, or related field is preferred
- Experience in prevention and/or recovery
- Good oral and written communication skills
- Command of all skills necessary to organize and manage multiple tasks
- Ability to communicate effectively with low-income, disadvantaged population
- Excellent understanding and usage of Microsoft programs including Work, Excel, Outlook and PowerPoint
