Job Summary
Provide case management to assigned caseloads by developing and monitoring individual support plans in collaboration with agency consumers. Assist consumers with problem solving, crisis intervention, and daily living skills through hands-on support and referrals. Accompany consumers on medical, psychiatric, and public benefit appointments while acting as an advocate to encourage self-advocacy. Conduct regular home and hospital visits, support Facilities Management staff in maintaining apartment standards, and assist with moves including lifting, packing, and cleaning. Liaise with community-based services, civic groups, and government organizations to ensure community inclusion and integration. Document all services in compliance with agency standards and participate in ongoing supervision, team meetings, and trainings. Adhere to an on-call cell phone system for immediate crisis intervention.
Required Qualifications
- Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System
- Must show proof of vaccination against the COVID-19 virus
Desired Qualifications
- Bachelor's Degree preferred, not required or 3 years of experience working with the behavioral health population
- 2 years supervisory experience
- Two years' experience with individuals with mental illness, substance use issues and/or a history of homelessness
- Experience in group work, tenant organization, case management, referral, and/or community outreach
- Experience in scattered site setting
- Experience or training in peer counseling or advocacy
- Bilingual English/Spanish speaking
