Job Summary
Provide case management to assigned caseloads by developing, monitoring, and updating individual service plans for agency consumers. Assist with problem solving, crisis intervention, and regular home or hospital visits while accompanying consumers on medical and public benefit appointments. Act as an advocate for consumers, address daily living skill deficits through hands-on assistance or referrals, and support Facilities Management in maintaining apartment standards. Conduct moves with consumers and staff, including lifting, packing, and cleaning. Liaise with community-based services, civic groups, and government organizations to ensure community inclusion. Document all services in compliance with agency standards and participate in supervision, team meetings, and trainings. Adhere to an on-call cell phone system for immediate crisis intervention.
Required Qualifications
- Bachelor's Degree
- Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System
Desired Qualifications
- 2 years supervisory experience
- Two years' experience with individuals with mental illness, substance use issues and/or a history of homelessness
- Experience in group work, tenant organization, case management, referral, and/or community outreach
- Experience in scattered site setting
- Experience or training in peer counseling or advocacy
- Bilingual English/Spanish speaking
