Wellbeing Worker

Deliver intake, assessment, referral, and support coordination for people experiencing severe and persistent mental illness within the CPSP in Gippsland. Conduct needs assessments, screen for NDIS eligibility, manage referrals/waitlists, facilitate access to clinical and community services, and provide tailored care planning and capacity-building group activities. Build partnerships with NDIS, Gippsland PHN, and community/clinical services; support housing stability and recovery through person-centred practice and a recovery-oriented framework. Key duties include intake/assessment, eligibility screening, referral management, waitlist prioritisation, group activities, data collection, and active waitlist management, with liaison to stakeholders and ensuring access to a range of psychosocial supports.” ,
