Job Summary
Consult with primary care physicians to enhance patient understanding, provide decision support for treatment planning, and assist in monitoring bio-psychosocial treatment plans. Conduct individual, couple, and family consultations including functional assessments, psychoeducation, medication adherence counseling, and motivational interviewing. Develop self-care plans and implement harm reduction strategies for substance use evaluation. Document assessments, progress notes, and billing as required. Maintain strict patient confidentiality and adhere to organizational policies and compliance standards. Requires current PA LCSW license, minimum two years clinical experience, and women's health expertise.
Required Qualifications
- Master's degree in social work or related field
- 2 years' experience as a therapist
- current license to practice in PA as a Licensed Clinical Social Worker
- Competency in providing cognitive and behavioral interventions to patients with behavioral health/substance abuse issues in a primary care setting
- Women's Health experience
