Provide direct one-on-one medical home health care to pediatric patients (infants, children, and adolescents) in clients’ homes; administer prescribed medications, treatments, and therapies; perform ongoing patient assessments; coordinate care with physicians and other personnel; educate family members on patient clinical care to improve outcomes; focus on pediatric home-based care with weekends and variable shifts (full-time or part-time); entry requirements include an active RN or LVN/LPN license; prior home health experience preferred.
