Job Summary
Provide skilled nursing care, assessments, and symptom management in hospice; complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB); respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care; collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes; educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
Required Qualifications
- Active RN License required
- Hospice nursing experience preferred
- Experience with Homecare Homebase (HCHB) preferred
- Strong assessment, communication, and care coordination skills
