Job Summary

Weekend Treatment Nurse (LVN) in a Skilled Nursing Facility responsible for providing and coordinating ordered treatments, wound care, and skin assessments, documenting care, and ensuring resident safety and comfort. Works with the Director of Nursing, charge nurses, physicians, and interdisciplinary team to ensure timely follow-up of treatment orders and changes in resident condition. Performs and documents treatments, wound care, skin assessments, measures and documents wounds, notifies appropriate team members of changes, reinforces treatment schedules, provides clinical guidance to CNAs and staff, and maintains timely, accurate electronic health records while adhering to infection prevention and HIPAA requirements. Requires California LVN license, CPR/BLS, and weekend availability.

Required Qualifications

  • Current and active California Licensed Vocational Nurse license in good standing
  • Current CPR/BLS certification
  • Minimum one year of nursing experience in a skilled nursing facility, long-term care, rehabilitation, wound care, or similar clinical setting preferred
  • Experience with wound care, skin assessments, treatment documentation, and electronic health records strongly preferred
  • Knowledge of skilled nursing regulations, infection prevention standards, resident rights, and documentation requirements
  • Ability to assess resident needs, recognize changes in condition, exercise sound clinical judgment, and escalate concerns appropriately
  • Strong organizational, time-management, communication, and interpersonal skills
  • Ability to work independently during weekend operations while collaborating effectively with the nursing team and on-call leadership

Desired Qualifications
