Weekend Start of Care (SOC) Nurse (RN)

Conduct Start of Care (SOC) nursing assessments, documentation, and direct patient care for new admissions and post-hospitalization cases. Develop care plans, assign Home Health Aides, and coordinate telehealth notifications while monitoring patient symptoms and reporting treatment reactions. Teach families on care needs, initiate preventive procedures, and communicate timely updates to the supervisor and home health team. Organize daily visit schedules to minimize travel time and meet weekly productivity targets of six points, including three SOC or ROC visits per day. Work weekends plus two weekdays, including holidays as required, while adhering to agency policies and completing CEU requirements.
