Job Summary
On-Call Weekend Nurse (RN or LPN) for Hospice/Reserve Care. Schedule: Saturday-Sunday shifts 8:00 AM–8:30 PM, providing intermittent skilled nursing in patients’ homes under physician direction. Responsibilities include assessing physical, psychosocial, and spiritual needs; delivering nursing care; coordinating with physicians; offering emotional support; patient/family education; documentation; and participation in team meetings. Requires Ohio RN/LPN license, high school diploma or equivalent, and ability to work independently in home settings; preferred BSN and experience in pain/symptom management, oncology, or mental health. Valid driver’s license and ability to travel to patient residences required.
Required Qualifications
- RN or LPN license in the state of Ohio
- High School Diploma or equivalent
- Graduate of an accredited school of nursing (RN or LPN)
- Ability to work independently and with a team
- Effective communication skills (oral and written)
- Ability to travel to patients' residences in Ohio
- Proficiency with MS Office (Word/Excel) and related team software
