Job Summary
Weekend RN Case Manager in Hospice provides nursing care to terminally ill patients in home or skilled-nursing settings, conducts regular nursing assessments, offers dietary counseling, initiates and performs hospice procedures, teaches and counsels patients and families, supervises nursing staff, develops and revises the patient-family care plan with the IDG, participates in team conferences, maintains documentation per HIPAA, and provides on-call coverage. Requires RN licensure, hospice/home-care nursing experience, strong assessment and care-coordination skills, and ability to work with an interdisciplinary team to ensure patient comfort and safety.
