Job Summary
Assess physical, psychosocial, and spiritual needs of patients and families, then gather relevant data and provide intermittent skilled nursing services under physician direction. Monitor and evaluate pain and symptoms while offering emotional support, health care teaching, and referrals as appropriate. Serve as a liaison to physicians and team members, relay pertinent information, and participate in mandatory trainings or triage coverage. Make visits to patient residences, including at the time of death, maintaining all required documentation. Work two 12-hour shifts on Saturdays and Sundays, requiring a valid driver's license and ability to travel in various weather conditions.
Required Qualifications
- High School Diploma or equivalent
- Licensed RN/LPN in state of Ohio
- Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer
- Ability to access patients and families in any residential and/or public setting in any weather condition
- Ability to stand and walk for extensive periods of time
- Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment
- This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics when working with patients
- Ability to stoop, squat, or bend frequently
- Corrected vision and hearing within normal range to observe and communicate with patients and professional staff
- Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
- Provide an active driver's license
- Ability to provide proof of eligibility to work in the United States
- High level of integrity, ethics and professionalism
- Must be able to make good professional decisions quickly despite stressful atmosphere
Desired Qualifications
- Graduate of an accredited school of nursing, with a Bachelors degree preferred
- Experience preferred in pain and symptom management, oncology, or mental health
- Ability to communicate effectively, orally and in writing
- Ability to work independently and collaboratively with a team
- Must have demonstrated knowledge and skills to provide appropriate care to all age groups
- Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology
