Job Summary
Assess physical, psychosocial, and spiritual needs of patients and families, then gather data to monitor and evaluate pain and symptoms. Provide intermittent skilled nursing services under physician direction, offer emotional support, and relay pertinent information to the care team. Serve as a liaison to physicians, provide health care teaching for residence-based care, and make visits to patient homes, including at the time of death. Maintain and promptly complete all required documentation while participating in triage coverage and team meetings. Work two twelve-hour shifts on Saturdays and Sundays with autonomy to utilize nursing skills in urgent after-hours situations.
Required Qualifications
- High School Diploma or equivalent
- Licensed RN/LPN in state of Ohio
- Must have and maintain a valid driver's license
- Good driving record
- Auto accident insurance at a level acceptable to HWR and/or HWR's insurer
- Corrected vision and hearing within normal range
- Ability to access patients and families in any residential and/or public setting in any weather condition
- Ability to stand and walk for extensive periods of time
- Manual and finger dexterity and eye-hand coordination
- Ability to stoop, squat, or bend frequently
- Ability to handle stressful and constantly changing conditions
- Ability to make good professional decisions quickly despite stressful atmosphere
- Ability to communicate effectively, orally and in writing
- Ability to work independently and collaboratively with a team
- Must have demonstrated knowledge and skills to provide appropriate care to all age groups
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
- Provide an active driver's license
- Ability to provide proof of eligibility to work in the United States
- High level of integrity, ethics and professionalism
Desired Qualifications
- Graduate of an accredited school of nursing, with a Bachelors degree preferred
- Experience preferred in pain and symptom management, oncology, or mental health
- Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology
