Job Summary
Conduct home health visits to adult and geriatric clients in Mecklenburg and Union counties, administering medications, wound care, and treatments per designated care plans. Perform client assessments, monitor conditions, and report changes to Clinical or Client Services Managers. Execute and document doctors' orders, observations, and interventions using a touch pad tablet for case management and coordination. Requires current NC RN license, one year of recent clinical experience, and prior home health experience.
Required Qualifications
- HOME HEALTH/OASIS DOCUMENTATION EXPERIENCE REQUIRED!
- A current license as a Registered Nurse in North Carolina.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home health experience required.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Ability to travel to cases as assigned.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
