Job Summary
Provide direct care to terminally ill patients every other weekend, administering nursing care, managing symptoms, and developing individualized care plans. Assess patient needs, coordinate with interdisciplinary teams, and direct end-of-life care while providing education and support to families. Maintain accurate medical records, supervise LPNs and CHHAs, and rotate on-call for after-hours triage, admissions, and death attendance. Drive a personal vehicle to visit patients in their homes or facilities across Yellowstone County and south-central Montana.
Required Qualifications
- Associate degree in Nursing
- Current license as a Registered Nurse in Montana
- Valid State of Montana Driver's License and vehicle insurance
- Current BLS certification
- Computer literacy, in Microsoft Office Suite
- Knowledge and understanding of protected sensitive patient health information (HIPAA) and confidentiality
- Ability to maintain a calm and positive demeanor during difficult interactions
- Ability to display non-judgmental and empathetic listening skills
- High degree of detail-oriented skill level
- Ability to prioritize and perform a wide range of tasks under pressure of continuous interruptions
- Ability to perform job duties with integrity and innovation to ensure completion and a high level of quality
- Ability to understand and adhere to required administrative policies and procedures
- Excellent communication skills
- Ability to travel and use own personal vehicle
- Ability to stand, walk and sit
- Ability to talk or hear, both in person and by telephone
- Ability to use hands to finger, handle or feel objects or controls
- Ability to reach with hands and arms
- Ability to stoop, kneel, bend, crouch and lift up to 25 pounds frequently and 50 pounds occasionally
- Specific vision abilities including close vision, distance vision, depth perception, color vision and the ability to adjust focus
- Risk of needlestick injury mitigation
- Protection against biological pathogens and contagious diseases
- Adherence to safety, security, and health requirements
- Independent professional judgement regarding patient health
- Maintenance of equipment and management of resources pertaining to the program budget
Desired Qualifications
- Bachelor of Science in Nursing
- Experience with hospice/palliative care
- Experience with home health care
