Weekend Registered Nurse

Weekend Registered Nurse responsible for delivering the Plan of Care for home health patients, initiating and coordinating care, and maintaining documentation. Responsibilities include performing initial nursing evaluations for hospice eligibility within 48 hours of referral, reassessing patient needs at least every 15 days, coordinating interventions, maintaining clinical notes, and collaborating with the patient/family, attending physician, and other IDG members to ensure quality hospice care. Required qualifications include active RN licensure, valid driver's license with access to a vehicle or reliable public transportation, CPR certification, and ability to work in diverse home settings. Preferred qualifications include the ability to work independently and strong communication, writing, and organizational skills. Salary ranges from $67,104 to $100,656 annually for full-time employment along with a comprehensive benefits package.
