Job Summary
Conduct head-to-toe assessments, triage urgent clinical situations, and manage acute and post-acute care for adult and senior patients in home settings. Perform wound care, IV therapy, and medication reconciliation while documenting OASIS Start of Care data and safety monitoring. Make real-time clinical decisions independently and escalate concerns when necessary, adapting to varied home environments and dynamic visit schedules. Requires current Nevada nursing license, Medicare Certified Home Health experience, and proficiency with point-of-care documentation technology.
Required Qualifications
- current nursing license in good standing in the state of Nevada
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- current or recent experience in a Medicare Certified Home Health setting
- OASIS (especially Start of Care) documentation
