Job Summary
Weekend Home Health Registered Nurse provides direct patient care in the patient's home under physician supervision. Responsibilities include start-of-care planning, wound care, medication administration, discharges, case management, developing treatment plans, submitting patient care progress reports, coordinating plan of care, and maintaining clinical documentation. Requires an Associate degree in Nursing, California RN license, CPR/BLS, and 2-3 years of Home Health experience; proficiency with EMR systems (WellSky/Kinnser a plus) and experience with wound care and IV therapy.
Required Qualifications
- Associate degree in Nursing (or higher)
- Current California RN license
- Current CPR/BLS certification
- Minimum 2-3 years of RN experience in Home Health within the past year
- Experience in wound care/wound vac, patient teaching, medication management, lab draws, and IV/TPN administration
- Ability to chart with Electronic Medical Record Software (Wellsky / Kinnser a plus)
