Job Summary
Weekend Home Health Registered Nurse provides direct patient care in the patient’s home under physician supervision. Responsibilities include creating and following start of care plans, wound care, medication administration, discharge planning, and collaborating with the clinical team to achieve positive patient outcomes. Requires an Associate degree in Nursing, current CA RN license, CPR/BLS, and 2-3 years of Home Health experience. Proficient in wound care, patient teaching, medication management, lab draws, IV/central line care, and EMR charting with systems such as WellSky or Kinnser. The role is part-time with weekend shifts (Saturday and Sunday) in San Bernardino, CA and surrounding areas.
Required Qualifications
- Minimum of Associate degree (or higher) in Nursing.
- Current and unrestricted California Registered Nurse license.
- Current CPR/BLS certification.
- Minimum of 2-3 years of RN experience in Home Health within the past year
- Experience in wound care/wound vac, patient teaching, medication management, lab draws, and Peripheral IV, PICC/Central Line, IV/TPN administration, CADD pump use, Foley Catheter, PleurX, Tracheostomy
- Ability to chart with Electronic Medical Record Software (Wellsky / Kinnser a plus)
