Job Summary
Weekend Home Health Registered Nurse provides direct patient care in the patient's home under physician supervision. Responsibilities include starting care plans, performing wound care, medication administration, discharges, and collaborating with a clinical team to achieve positive patient care outcomes. Responsibilities also include case management & follow-up nursing visits, developing treatment plans based on physician orders, submitting patient care progress reports, discharge planning, care documentation, and coordinating resources and referrals. Requires CA RN license, CPR/BLS, associate degree or higher, 2-3 years home health RN experience, wound care and IV/central line care, and EMR proficiency (Wellsky/Kinnser). Must work Saturdays and Sundays.
Required Qualifications
- Minimum of Associate degree (or higher) in Nursing
- Current and unrestricted California Registered Nurse license
- Current CPR/BLS certification
- Minimum of 2-3 years of RN experience in Home Health within the past year
- Experience in wound care/wound vac, patient teaching, medication management, lab draws, and Peripheral IV, PICC/Central Line, IV/TPN administration, CADD pump use, Foley Catheter, PleurX, Tracheostomy
- Ability to chart with Electronic Medical Record Software (Wellsky / Kinnser a plus)
