Job Summary
Weekend Home Health Registered Nurse provides direct patient care in patients' homes under physician supervision; responsibilities include following start of care plans, wound care, medication administration, discharge planning, and collaborating with a clinical team to achieve positive patient outcomes. Must perform case management & follow-up skilled nursing visits, develop treatment plans based on physician orders, document patient progress, and coordinate resources. Requires California RN license, CPR/BLS, and 2-3 years of home health experience, with proficiency in wound care, IV/central line care, medication management, lab draws, and EMR/charting.
Required Qualifications
- Minimum of Associate degree (or higher) in Nursing
- Current and unrestricted California Registered Nurse license
- Current CPR/BLS certification
- Minimum of 2-3 years of RN experience in Home Health within the past year
- Experience in wound care/wound vac, patient teaching, medication management, lab draws, and Peripheral IV, PICC/Central Line, IV/TPN administration, CADD pump use, Foley Catheter, PleurX, Tracheostomy
- Ability to chart with Electronic Medical Record Software (Wellsky / Kinnser a plus)
