Job Summary
Conduct initial, comprehensive, and ongoing assessments of residents to determine medical needs. Write, initiate, and revise the plan of care as required. Deliver preventative and rehabilitative nursing care while administering medications and treatments per prescription. Complete, maintain, and submit accurate clinical notes regarding resident conditions and care provided. Ensure confidentiality of all resident care information and promote a culture of safety within the living environment. Maintain current licensure and demonstrate strong communication and organizational skills to support high-quality, person-centered care in a family-oriented Skilled Nursing Facility.
Required Qualifications
- RN with current licensure to practice in the state
- Comprehensive knowledge of applicable nursing principles and practices to ensure the quality of care standards for residents
- Strong communication and organizational skills
- Self-directed and able to work with minimal supervision
Desired Qualifications
- Experience in a Skilled Nursing Facility (SNF) environment
- Nursing experience providing direct care to multiple residents
- Behavioral care / memory care experience
- Experience with PointClickCare (PCC)
