Job Summary
Conduct patient assessments of functional status, mobility, endurance, and pain to determine treatment plans and progress. Perform interventions to relieve pain, restore function, and maintain maximum functioning within state-specific guidelines. Instruct patients and caregivers on care plans and functional limitations while reporting treatment responses and condition changes to supervising staff. Visit patients according to the plan of treatment, complete progress notes per policy, and submit evaluations detailing goals and projected care frequency.
Required Qualifications
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
- Candidates are required to pass a drug test before beginning employment
