Job Summary
Conduct home health visits for adult and geriatric clients across Wake and Durham counties, performing diagnostic tests for mobility, balance, and muscle strength. Implement individualized care plans including manual therapeutic exercises and gait training while assessing and revising plans with nursing. Educate clients and family members on rehabilitative care and safety, then accurately document all observations and interventions using a touch pad tablet. Requires a current North Carolina PT license, one year of recent experience, and OASIS documentation proficiency. Available for two weekends per month in Raleigh, NC.
Required Qualifications
- Must be available 2 weekends per month
- Home Health/OASIS documentation experience
- A current North Carolina Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
