Job Summary
Conduct weekend home health visits to adult and geriatric clients across Pima County, performing diagnostic tests for mobility, balance, and strength while implementing integumentary protective interventions and wound repairs. Develop and revise individualized care plans featuring manual therapeutic exercises, gait training, and balance interventions, then educate clients and family members on rehabilitative activities. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet as part of a multidisciplinary team providing skilled nursing and rehabilitative care. Requires a current Arizona Physical Therapist license, one year of recent clinical experience, and ability to work independently.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- Current AZ Physical Therapist - PT license
- Minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills
- Prior experience with electronic medical records (EMR)
- Must be available for weekend home health visits
- Must be able to perform diagnostic tests and measurements
- Must be able to implement integumentary protective interventions
- Must be able to develop and implement appropriate individualized care plans
- Must be able to accurately document observations, interventions, and evaluations
- Must be able to educate and instruct clients, family members, or other client representatives
