Job Summary
Assist with referral intake and processing through dispatch using multiple technology platforms, supporting psychosocial assessment of complex and challenging referrals. Follow up on referrals to ensure access, quality, and timeliness while participating in quality improvement initiatives. Train peers on referral/intake/dispatch processes and end-of-life topics. Work Saturdays and Sundays from 8:00am to 8:30pm for 24 hours, paid for 28 hours, as a Licensed Social Worker in Ohio. Join Reserve Care to deliver expert hospice, palliative, and grief support with a focus on relieving suffering and enhancing comfort.
Required Qualifications
- Licensed Social Worker (LSW) in the state of Ohio
- Ability to provide proof of eligibility to work in the United States
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
- Provide an active driver's license
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
- High level of integrity, ethics and professionalism
- Ability to work effectively within an interdisciplinary team
Desired Qualifications
- Hospice experience preferred
