Job Summary
Conduct telephonic and in-person assessments for participants within the LIFE Pittsburgh Adult Day Health Centers. Manage on-call phone communications to determine best courses of action, provide direct care including triage, administer treatments, and coordinate comprehensive assessment plans. Notify physicians of changes in patient conditions, medication errors, or abnormal lab results. Complete all necessary documentation according to regulations and policies. Requires a valid PA Registered Nurse License, CPR certification, and a vehicle for travel between centers.
Required Qualifications
- Valid PA Registered Nurse License
- Valid CPR certification
- Valid Driver's license and automobile insurance
- Must have a vehicle for travel
- Graduate of an accredited school of nursing
- Minimum (1) year experience working with the frail or elderly
- Experience working with and relating to the geriatric population and their family members
- Ability to obtain and interpret information in terms of the specific needs of the Participant
- Frequently required to manage many details within a fast paced environment
- Possesses knowledge of the Human Anatomy and development of the Participant served
- Understands the range of treatment needed by the participant served
- Well-developed organizational and interpersonal skills
- Strong written and verbal communication skills
- Ability to multi task and problem solve complex situations
- Must be flexible and able to change easily
- Ability to work with people from various social- cultural backgrounds effectively
