Job Summary
Assess and identify patient and family needs, then plan, implement, evaluate, and document individualized hospice nursing care in accordance with the interdisciplinary plan of care. Coordinate care with the interdisciplinary group and other medical providers while supervising Licensed Practical Nurses and Certified Nursing Assistants. Conduct admission visits for medically eligible patients, follow up on new admissions, and perform facility-to-home transfers as needed. Assigned patients may reside in homes, skilled nursing facilities, assisted living facilities, or acute care settings. Requires current RN licensure in Kentucky or Indiana, a valid driver's license, and familiarity with electronic medical records. Shifts run Friday through Sunday evenings.
Required Qualifications
- RN with current RN licensure in the state of Kentucky and/or Indiana as required by designated position, or the ability to obtain required license/s upon offer of position
- Must have a valid driver's license
Desired Qualifications
- BSN desired
- Certification in hospice/palliative care preferred
- One year of nursing experience preferred
- Knowledge of palliative care helpful
- Use of technology-based tools, including electronic medical record, strongly preferred
