Job Summary
Weekend Licensed Practical Nurse (LPN) – Home Health. Responsibilities include providing individualized nursing care to residents using the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation), collaborating with residents, families, and inter-disciplinary teams to plan and evaluate care, and potentially serving as charge nurse with delegation to licensed/unlicensed personnel. Requires current nursing licensure and completion of an accredited nursing program; RN licensure in applicable states may be required for telephone nursing. BSN preferred; experience in long-term care, assisted living, and dementia care encouraged. The role supports full-time scheduling; weekend work is not required. Required Qualifications
- Must have graduated from an accredited school or college of nursing
- Current licensure as a Registered Nurse in the state, or states, of practice (for nursing practice over the telephone may require licensure in patients' states)
- Bachelor's degree in nursing is preferred
- 2? minimum experience note: 'minimum of 400 hours of experience as a RN (or LPN) at another facility'
