Weekend Hospital Liaison (RN)

Job Summary
Assess patient eligibility, coordinate hospital discharge planning, and facilitate referrals to hospice services by evaluating appropriateness for program services and communicating with primary physicians. Maintain a hospice presence in hospitals and long-term care facilities by coordinating with providers, case managers, and referral sources to foster connections and increase referral volume. Assist with the referral intake process by providing program information, collecting data, and developing referrals according to agency procedures. Requires a current NC RN license, two years clinical experience, and valid driver's license. Flexible scheduling available for weekend coverage or full-time hours with additional shifts.
Required Qualifications
- Graduate of accredited school of nursing – Associates Degree
- Two years clinical nursing experience
- Current North Carolina Registered Nurse license
- Valid NC driver's license
- Reliable independent transportation
- Current automobile liability insurance at limits required by the agency
- Strong working knowledge of hospice eligibility criteria
- Ability to maintain positive working relationships and courteous interaction with public
- Ability to work a flexible schedule
- Excellent verbal and written communication skills
- Excellent interpersonal and customer service skills
- Proficient in Microsoft Office Suite or related software
- Excellent organizational skills and attention to detail
- Ability to work independently and in a team environment
