Job Summary
Provide home-based care to patients with six months or less to live, focusing on quality of life and administering prescribed medications and treatments. Perform patient assessments, check vitals, monitor pain, and evaluate breathing as part of an interdisciplinary team carrying out the patient's plan of care. Educate families on prognosis, care plan changes, and hospice goals while ensuring accessibility of medical equipment and supplies. Take a holistic approach that engages the patient's mind, body, spirit, and family. Maintain 1:1 nurse-to-patient ratios with flexible assignments and autonomy. Weekend PRN position with market competitive visit rates, mileage reimbursement, and online training for CEUs.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Indiana
- Minimum of 1 year of nursing experience
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty
- Strong communication skills
- Compassion
- Weekend PRN position
