Job Summary
Conduct weekend home health visits for adult and geriatric clients in Dartmouth, Somerset, New Bedford, and Fairhaven, performing head-to-toe assessments, triage, and independent management of urgent clinical situations. Deliver comprehensive wound care, medication reconciliation, patient teaching, and safety monitoring while documenting findings at the point of care. Utilize acute and post-acute care expertise to make real-time clinical decisions in varied home environments, prioritizing urgent visits within a dynamic schedule. Requires one year of recent clinical experience in adult/geriatric settings with a strong preference for prior home health visiting experience.

- Licensed RN
- Current nursing license in good standing in the state of Massachusetts
- Graduation from an accredited and approved nursing program
- At least one year of current or recent hands-on medical experience within the past two years
