Job Summary
Conduct home health visits to adult and geriatric clients in Dedham, Newton, Needham, Cambridge, and Boston, following designated care plans and administering medications, wound care, and treatments. Perform client assessments, monitor conditions, and report changes to Clinical or Client Services Managers while accurately documenting observations, interventions, and evaluations via touch pad tablet. Execute and document doctors' orders, manage case coordination, and ensure compliance with state/local mandates. Required Qualifications
- One year recent prior clinical experience
- A current license as a Registered Nurse in Massachusetts
- Graduation from an accredited and approved nursing program
- Must be able to perform home health visits
- Must be able to administer medication
- Must be able to perform wound care
- Must be able to perform treatments and procedures
- Must be able to monitor clients' conditions
- Must be able to report changes to Clinical or Client Services Manager
- Must be able to follow up with, execute, and properly document doctors' orders
- Must be able to perform client assessments
- Must be able to handle case management and coordination
- Must be able to accurately document observations, interventions, and evaluations
- Must be able to utilize a touch pad tablet
