Job Summary
Conduct weekend home health visits to adult and geriatric patients in Milford, Medway, Franklin, and surrounding towns. Perform diagnostic tests and measurements for mobility, balance, strength, and ambulation patterns. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and safety assessments. Continually revise care plans in collaboration with nursing staff and educate clients and family members on rehabilitative activities. Accurately document observations and interventions using a touch pad tablet. Prior home health experience is strongly preferred. Base pay ranges from $64.00 to $72.00 per point with a weekend differential, plus a $5,000 sign-on bonus.
Required Qualifications
- A current license as a Physical Therapist in Massachusetts
- A minimum of one year of recent clinical experience as a licensed Physical Therapist
