Weekend Home Health Physical Therapist

Conduct home visits to adult and geriatric patients in Marlborough, Framingham, Natick, and surrounding towns. Perform diagnostic tests measuring mobility, range of motion, transfer status, balance, and muscle strength. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and safety assessments. Continually revise care plans with nursing staff and educate clients and families on rehabilitative activities. Accurately document all observations and interventions using a state-of-the-art touch pad tablet. Prior home health experience is strongly preferred, with a Massachusetts PT license and one year of recent clinical experience required.
