Job Summary
Weekend Home Health Care Transition Coordinator role requiring RN licensure and weekend coverage (Saturday and Sunday with flexibility to include Fri or Mon shifts). Coordinates transitions for patients into home health, home infusion, and hospice, supporting physicians, hospital case managers, and referral sources. Manages in-person bedside transitions, educates families about benefits, aligns post-acute care needs with care teams, and facilitates communication across clinicians. Requires strong communication, ability to interpret hospital records and physician orders, and knowledge of post-acute/post-discharge planning, home health eligibility, and end-of-life practices. Emphasizes patient-centered goals, adherence to Compassus values, and leveraging data analytics to improve outcomes while maintaining exceptional customer service and professional standards.
Required Qualifications
- Two to three years of nursing experience as an RN required
- Active and unencumbered Registered Nurse license in the state(s) of employment required
- Experience with home health eligibility admission requirements, COPs, PDGM knowledge and training, risk scoring/data analysis, introduction to end-of-life practices/spiritual history, homebound status determination, palliative care, General Dx and LCDs, and estimating and communicating prognosis preferred
