Job Summary
Assist patients with personal activities of daily living, oral hygiene, transfers, and ambulation while promoting resident self-direction. Perform shift-to-shift rounding, catheter care, ostomy bag application, and skin care as directed by the RN. Prepare rooms for new admissions, manage laundry, and accurately complete all documentation before shift leave. Answer call bells promptly, observe patient physical and emotional status, and participate in emergency preparedness and infection control protocols. Adhere to attendance standards, dress code, and in-service education requirements.
Required Qualifications
- Knowledge of skills required to provide excellent care in accordance with the role of Certified Home Health Aide (CHHA) or Certified Nurse Aide (CNA) in a hospice environment
- Excellent interpersonal skills for interacting with patients and families, nurses and employees, and patients and families
- Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing
- Visual acuity sufficient to read a thermometer and auditory acuity sufficient to hear and comprehend normal speaking tones, and to hear call bells with or without assistive devices
- Well organized to manage time, prepare care-related documentation
- Ability to complete a minimum of 12 hours of in-service education per year
- High School diploma or equivalent
- Must possess current Home Health Aide Certification by the New Jersey Board of Nursing, or a Certified Nurse Aide approved by the New Jersey State Department of Health and Senior Services
- Prior experience working in home care/hospice environment
