Job Summary
Retrieve, monitor, and process referrals for hospice services from multiple sources including CarePort, fax, and phone. Utilize the dispatch process to schedule appointments for prospective patients and follow up on stalled referrals. Perform direct admits to inpatient care centers as needed and take clinical calls from patients and caregivers to attempt resolution or escalate appropriately. Notify primary care teams of any clinical issues addressed and participate in quality improvement initiatives. Work Saturday and Sunday shifts from 8:00am to 8:30pm as a Licensed Practical Nurse in Ohio.
Required Qualifications
- Licensed Practical Nurse (LPN) in the state of Ohio
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
- Provide an active driver's license
- Ability to provide proof of eligibility to work in the United States
- High level of integrity, ethics and professionalism
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
Desired Qualifications
- Hospice experience strongly preferred
- Excellent verbal and written communication skills
- Ability to communicate calmly and compassionately with patients and families in distress
- Strong teamwork and collaboration skills
- Comfortable using multiple technologies, including: Electronic Medical Records (EMR), Phone systems, Referral and intake platforms
