Job Summary
Perform comprehensive patient assessments including OASIS assessments at required intervals, develop and revise individualized plans of care, and deliver skilled nursing services as outlined. Counsel patients and families on disease processes, self-care techniques, and prevention strategies while collaborating with the interdisciplinary team to ensure seamless, holistic care. This part-time weekend role requires full Saturdays and Sundays (20 hours per week) with willingness to participate in weekend and holiday on-call rotations. Required Qualifications
- Graduate of an approved nursing program with an active Florida RN license
- Minimum of 1 year of direct patient care experience
- Home Health RN experience
- Willingness to participate in weekend and holiday on-call rotations
