Job Summary
Conduct home health visits to adult and geriatric clients in Chester County, performing assigned duties including medication administration, wound care, treatments, and procedures. Follow designated care plans while monitoring client conditions and reporting changes to Clinical or Client Services Managers. Execute and document doctors' orders, perform client assessments, and manage case coordination using a state-of-the-art touch pad tablet. Maintain accurate documentation of observations, interventions, and evaluations. Requires a current Pennsylvania RN license, minimum one year of recent clinical experience, and ability to travel to cases as assigned. Weekend schedule available Friday-Sunday or Saturday-Monday.
Required Qualifications
- Weekend schedule would be Friday - Sunday OR Saturday - Monday
- Home health experience required
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience strongly preferred, but not required
- Prior experience with electronic medical records (EMR) preferred
