Job Summary
Complete home health admissions and Start of Care assessments, including comprehensive medical histories, physical evaluations, and safety checks. Develop individualized plans of care, educate patients and caregivers, and coordinate services with physicians, therapists, and interdisciplinary team members. Ensure compliance with federal regulations and maintain accurate OASIS documentation. This role serves as the primary admissions nurse Friday through Monday in Columbus, Ohio. Aspire Home Healthcare of Ohio offers a supportive environment focused on patient-centered care where individuals remain safe and independent in their homes.
Required Qualifications
- Current and active Registered Nurse (RN) license in the State of Ohio
- Minimum of one year of nursing experience
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Automobile insurance
- Current TB test
- Physical examination in accordance with agency requirements
- Ability to attend orientation and training at the Aspire Home Healthcare of Ohio office
- Friday – Monday schedule
