Job Summary
Conduct home health visits to adult and geriatric clients in Independent and Assisted Living communities throughout Cuyahoga County. Perform diagnostic tests including mobility, range of motion, transfer status, strength, balance, and safety assessments. Implement integumentary protective interventions for wound prevention and repair using evidence-based dressings. Develop and revise individualized care plans involving manual therapeutic exercises, gait training, and balance interventions while collaborating with nursing on multidisciplinary care. Educate clients and family members on rehabilitative care to promote health and independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current OH PT license, one year of recent PT experience, and graduation from an accredited program. Base pay ranges from $63.00 to $73.00 per point.
Required Qualifications
- Home Health Experience
- A current OH Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
