Job Summary
Perform weekend Start of Care (SOC) admissions with complete OASIS documentation and develop care plans in collaboration with the interdisciplinary team. Evaluate patient needs, coordinate necessary services, and educate patients and caregivers on disease management and home care best practices. Work across the metro in client homes, utilizing clinical judgment to help patients begin their care journey. Compensation includes $200 per visit plus mileage reimbursement for consistent weekend scheduling.
Required Qualifications
- Valid Minnesota RN license
- CPR certification
- Proven experience in home health
- OASIS documentation skills
- Reliable transportation
- Driver's license
- Active insurance
- Strong communication skills
- Clinical assessment skills
- Weekend-only scheduling availability
