Job Summary
Complete weekend Start of Care (SOC) admissions in patients' homes, performing comprehensive nursing assessments and OASIS documentation. Develop individualized plans of care by identifying clinical, medication, safety, and disease-management needs. Educate patients and caregivers on diagnoses, symptoms, and home care protocols while coordinating services with the interdisciplinary team. Submit accurate electronic documentation for each visit. This role offers $200 per admission plus mileage reimbursement for experienced home health nurses seeking weekend income. Candidates must hold an active Minnesota RN license, current CPR certification, and valid driver's license. Prior home health experience and strong OASIS knowledge are required.
Required Qualifications
- Active Minnesota Registered Nurse (RN) license
- Current CPR certification
- Previous home health experience
- Strong working knowledge of OASIS and Start of Care admissions
- Excellent clinical assessment and documentation skills
- Confidence working independently in patients' homes
- Valid driver's license
- Active auto insurance
- Reliable transportation
- Weekends Only availability
